For your first visit, please bring:

• Patient Registration Form.  To save time, you may click here to download the form  and complete the form prior to your visit.
• Insurance Cards
• List of all patient medications.  Antihistaminines will interfere with allergy testing and should be discontinued 72 hours prior to the appointment if allergy testing is a possibility
• Physician Referral if required by your insurance plan.
• Print Privacy Notice to review and bring to office visit
